The healthcare sector is increasingly expected to contribute to global sustainability goals. Hospitals, manufacturers, regulators, and patients all recognise that reducing the environmental footprint of healthcare is no longer optional; actually, it is becoming a strategic and societal necessity. Medical devices, however, present a unique challenge. Unlike many consumer products, they cannot simply be redesigned using more sustainable materials or manufacturing processes without first demonstrating that these changes do not compromise patient safety.
This creates one of the greatest dilemmas currently facing the medical technology industry: how can manufacturers innovate sustainably while complying with stringent regulatory requirements designed to protect patients?

Sustainability is Becoming a Regulatory Priority
Healthcare accounts for approximately 4–5% of global greenhouse gas emissions, with medical devices contributing significantly through manufacturing, transportation, packaging, sterilisation processes, and end-of-life disposal. Single-use plastics, complex electronic components, and energy-intensive production methods have attracted increasing attention from policymakers.
Governments worldwide are responding through initiatives such as the European Green Deal and Circular Economy Action Plan; Extended Producer Responsibility (EPR) schemes and waste reduction programmes; Ecodesign initiatives and Sustainable Products Regulation proposals, and others.
For medical device manufacturers, sustainability is therefore no longer solely a corporate social responsibility initiative; it is rapidly becoming a regulatory and commercial expectation.
Unlike many industries, medical technology operates under rigorous regulatory frameworks where even seemingly minor product modifications may require extensive regulatory assessment.
A manufacturer wishing to replace virgin plastic with recycled polymers; introduce biodegradable materials; reduce packaging; change sterilisation methods; redesign components to improve recyclability may trigger regulatory obligations requiring verification, validation, risk assessment, and in some cases, submission of substantial evidence to regulatory authorities or notified bodies.
The challenge is that environmental improvements can unintentionally affect several different sectors as well, including:
- Biocompatibility;
- Mechanical performance;
- Shelf life;
- Sterility assurance;
- Chemical stability;
- Usability;
- Cybersecurity (for connected devices);
- Compatibility with existing clinical workflows.
Consequently, sustainability initiatives cannot simply be treated as manufacturing improvements, they should be understood as regulatory projects.
The Regulatory Bottleneck
Current medical device regulations were primarily developed with one overarching objective: protecting patient safety and ensuring clinical performance. It remains fundamental and should never be compromised.
However, sustainability considerations were largely absent when many regulatory frameworks were developed.
For example, under the EU Medical Devices Regulation (MDR) and In Vitro Diagnostic Medical Devices Regulation (IVDR), manufacturers must demonstrate that devices continue to meet the General Safety and Performance Requirements whenever significant changes are introduced.
A sustainability-driven modification may therefore require:
- Updated risk management documentation;
- Additional verification testing;
- Biocompatibility evaluations;
- Usability assessments;
- Revised technical documentation;
- Notified body review;
- Post-market surveillance considerations.
Depending on the nature of the change, these activities can significantly increase development timelines and costs.
The result is a regulatory bottleneck where environmentally beneficial innovations may be delayed, not because they are unsafe, but because existing regulatory pathways were not designed with sustainability in mind.
Thus, regulators are increasingly acknowledging sustainability, although implementation remains fragmented.
Several developments illustrate this shift, such as:
- Packaging Reduction: Manufacturers are exploring reduced packaging volumes while maintaining sterility and transport protection;
- Reprocessing and Reusable Devices: Many healthcare systems are reassessing reusable devices to reduce waste, although this raises questions regarding cleaning validation, infection prevention, and lifecycle management;
- Environmentally Preferable Materials: Interest continues to grow in recycled plastics, bio-based polymers, and alternative materials. However, demonstrating long-term safety and performance remains a significant scientific challenge.
- Chemical Sustainability: Restrictions on substances such as PFAS and other hazardous chemicals are beginning to influence material selection across healthcare products.
- Sustainable Procurement: Public healthcare systems increasingly include environmental criteria within procurement decisions, encouraging manufacturers to demonstrate measurable sustainability performance.
Medical device manufacturers now face competing priorities:
- Ensuring patient safety;
- Maintaining regulatory compliance;
- Achieving commercial competitiveness;
- Responding to customer sustainability expectations;
- Meeting ESG commitments;
- Preparing for future environmental legislation.
This balancing act requires sustainability to become integrated into quality management systems rather than being treated as a separate environmental initiative.
Cross-functional collaboration between regulatory affairs, quality assurance, research and development, clinical teams, environmental specialists, and supply chain professionals is becoming essential.
Opportunities for Regulatory Innovation
Many industry stakeholders believe that regulatory frameworks could evolve to better support environmentally beneficial innovation without reducing patient protection.
Potential approaches include:
- Clearer regulatory guidance for sustainability-related modifications;
- Harmonised expectations across jurisdictions;
- Risk-based pathways for low-risk environmental improvements;
- Internationally recognised sustainability standards for medical devices;
- Increased dialogue between regulators, notified bodies, manufacturers, and healthcare providers.
Some regulatory authorities have already begun discussing sustainability as part of broader healthcare policy, although comprehensive frameworks remain under development.
The future of medical technology will require a careful balance between two equally important objectives: protecting patients and protecting the planet.
Patient safety must remain the foundation of every regulatory decision. At the same time, environmental sustainability cannot remain an afterthought if healthcare systems are to contribute meaningfully to global climate goals.
Rather than viewing regulation and sustainability as competing priorities, the industry has an opportunity to develop a new regulatory paradigm where environmental performance becomes an integrated component of product quality, lifecycle management, and innovation.
Achieving this balance will require collaboration among regulators, manufacturers, healthcare providers, and policymakers. Those organisations that proactively embed sustainability into their regulatory strategies today are likely to be better positioned for the evolving expectations of tomorrow’s healthcare landscape.
